TRAVEL VACCINATIONS ADMINISTERED AT SUNNISIDE SURGERY
1) Visit a local travel vaccination pharmacy clinic such as Boots or Superdrug.
2) Gather documented advice. This MUST be provided, or the nursing team cannot vaccinate if needed.
3) Hand the documented information to the surgery reception team at least 6 weeks before travel (this allows time to book and attend an appointment). Please provide name, DOB, address and contact number also.
4) The clinical team will look at your records and assess the form. They will contact you if you require any of the four free NHS travel vaccines available to us. They are as below:
- POLIO (COMBINED)
- TYPHOID
- HEP A
- CHOLERA
Any other travel vaccines that have been recommended by the travel clinic and are not included in the four above must be administered by a private provider such as a travel clinic and are at a cost to yourself.
